It is barrel organ. I think that the keyboard of the barrel organ very well is fit for a Hint category.
It is possible to meet the organ-grinder which is playing this barrel organ on Karol's Bridge in Prague.

A nice piece of musical instrument that is really a one-man-band! I have never seen one in actual and this picture is a treat to my eyes. TFS.
