I've run the shot in Neat Image to reduce the noise...adjusted a bit the settings and voila ;) It gives a smoother skin tone. I hope you like it. It look almost like a painting I suppose. I still like your original more actually.
Just to show you what I liked while working on the image. I played with the filters, and I liked this 'old newspaper' filter, as I think it gave the pic a timeless feeling.
Changed the image to grey scale and left only the clothing item in colour. Like I said before not an improvement just to look at it differently. I hope you like it!

Must seem like I am a big fan of sepia tones. But I feel they do give a more moody feel to the picture. Since mood is so prominent in yours. The black on the sailors shirt might be a deterrent but with a frame I guess that should not be a problem. I hope you like this one.
